Groups Application

Thank you for taking the initiative to lead a group here at Bayside!

Registering your group using this application, allows us to see the details for when and where you want to form the group as well as the type of group you intend to lead.  Please use the description area to write an invitation for those considering your group.

Group Leader Requirements
For anyone to lead a group or team at Bayside, we ask that you complete the following. If you have not completed these, it will delay the approval of your group.
  • the Growth Track (My Bayside, My Family & My Freedom).
  • Participate in a Freedom Group and the subsequent Freedom event. 
  • The Bayside Leader Training video course and the follow-interview. 
  • Sign the Leader Covenant.
  • For Freedom Group leaders, you should have been asked to shadow a group before leading your group.

If you have co-leaders, they also must complete the requirements.

Group Information

Please write your description as if inviting someone to join. This will be cut and pasted into the Group Description box, so make sure you proof it!

Include required or optional items you would like members to bring. You can add links!
No need to include time and location.

Also, do not include private phone numbers, emails, or gate codes. You will have the opportunity to provide additional messaging to your group members through the events set up.

If this is for a Freedom Group, simply put Freedom in the Description.

For co-ed groups, there must be a male and female leader.

An open group is available for anyone to register. A private group is by invitation only from the group leader.

Schedule Information

Groups will not be processed without day, time, and location information. If you need assistance, please contact the Groups Director at your campus.

The Groups ministry no longer offers a monthly option. A group should meet at least twice a month to build community among its members.

The day you anticipate starting. This is not set in stone, in fact as a leader you set up your own schedule! However, this does help the staff process the group.

Confidential Information Access Agreement

Data breaches that affect the security of people’s identity are becoming more widespread. Because the safety of all of our attender data is important to us, we only grant access to this data when absolutely necessary. Once we receive this form, we will evaluate and ask further questions as needed before making a decision. Volunteers must maintain the confidentiality of all trade secrets and other confidential information of the Church, its employees, church members, contributors and volunteers that is entrusted to them, except when disclosure is legally mandated or expressly authorized by the Church. "Confidential information," for this purpose, includes any proprietary or other nonpublic information of the Church, or of other entities or organizations, or of any of the church members, contributors and volunteers that, if disclosed to persons not authorized to receive it, would be harmful to the relevant individual, entity or organization or detrimental to the Church's interests. Any volunteer who believes that any other confidential information must be disclosed because of legal requirements must consult with a Senior Associate Pastor and receive written confirmation from Bayside’s legal counsel that his or her understanding of the law is correct, before any such information is disclosed. PLEASE NOTE: In order to gain access to Planning Center, the volunteer must have completed the Growth Track at Bayside.

In addition to the statement above, I understand and acknowledge that:

• My login is to be used by me only and may not be shared.
• I may not log in to my account for somebody else to use.
• I may not allow unapproved people to view my screen or any printed reports.
• My access is for supervisor-approved tasks only. I may not access information about people or ministries that are not required for my assigned task.
• I may not provide confidential information to others without permission from my supervisor.
• Accurate and complete data must be entered at all times including use of capital letters and proper spelling.
• My access will be suspended after 60 days of inactivity or if I’m in violation of any of these policies.
